It’s a common metaphor in military parlance for the front line or “spearhead” of an assault.
The Striker subclass questline includes a quote from Kabr:
“What the City wants us to do—what we want to do—it doesn’t make any damned sense. That phrase ‘tip of the spear’ sounds good in speeches, but you know what happens to spear tips? They break. But that’s what it takes. If the choice is between you or the Wall? You break.” —Kabr
And in the mission “Queen’s Ransom”, Petra Venj uses the phrase in its typical context.

I thought that’s where I heard it from.
Actually, the main perk of the Invective have the Cassoid’s symbol, so probably it was created by the weapon foundry Cassoid, the one which seems to have created Telesto, and was mentioned in the sword quests.
